CITY OF ELYRIA TO HOST FEDERAL PRESS CONFERENCE ON AFFORDABLE CARE ACT INSURANCE ENROLLMENT

Federal Health and Human Services Region V Director Kathleen Falk will visit Elyria, Ohio on Friday March 14th at 1:00pm at Elyria City Hall  – 131 Court Street, Suite 301 to hold a press conference  to promote insurance enrollment under the provisions of the Patient Protection
& Affordable Care Act.   Falk will be joined by Elyria Mayor Holly Brinda and healthcare professionals from around Lorain County in an effort to maximize enrollment.

New national and state enrollment data will also be released, using the City of Elyria as a backdrop to describe insurance enrollment opportunities and challenges for the uninsured and under-insured around the United States.

“We are very pleased that the City of Elyria was chosen as the site in Northeast Ohio to help tell this extraordinary story of opportunity for so many individuals that otherwise would not have access to affordable, high quality healthcare,” said Mayor Holly Brinda.

Katheen Falk was appointed to her position in 2013 and brings many years of experience and service as a government leader in the State of Wisconsin, serving previously as County Executive in Dane County, Assistant Attorney General in the Wisconsin Department of Justice,  and Co-Director of a non-profit organization called Wisconsin’s Environmental Decade.  A native of Wisconsin, Ms. Falk earned a Bachelor’s Degree from Stanford University and her Law Degree from the University of Wisconsin Law School.

Those who would like to enroll can also walk in or preferably make an individual appointment (440- 240-1665) at one of three Lorain County Health & Dentistry locations including:

  • 1205 Broadway Avenue – Lorain, Ohio  44052
  • 3745 Grove Avenue – Lorain, Ohio  44055
  • 412 East River Road – Elyria, Ohio  44035

Individuals who attend these events or make an appointment and want to enroll must bring
the following documents:

  • Social Security Number
  • Birth Date
  • Pay Stubs, W-2 Forms, or Wage and tax Statements
  • Policy Numbers for any current health insurance
  • Information about health insurance available through current employer (if applicable)

The deadline for enrollment in the Affordable Care Act is March 31, 2014.  For more information contact your local health department or call (440) 240-1655 to speak with a Certified Application
Counselor.

Notice of Public Meeting and Information Repository for a Clean Ohio Assistance Grant

The City of Elyria is applying for a grant from the Clean Ohio Assistance Fund for a Phase II Environmental Assessment of the General Industries property located at 154 Olive Street, Elyria, Ohio.  The application will be available from March 10 through April 28 for review at the Elyria Central Library located at 320 Washington Ave. Elyria, Ohio 44035, Phone: 440-323-5747.  A public meeting to discuss and solicit comments to the grant application will be held on April 25, 2014 at Elyria City Hall Council Chambers located at 131 Court Street, Elyria, Ohio  at 4:00 p.m.   Application information is also available online at http://www.cityofelyria.org .  Any questions may be referred to Timothy J.
